*Forms GS2 and GS4 are accepted through the deadline dates listed above, however, to avoid the assessment of late fees, students should submit these forms within the time frame suggested in the Graduate School Announcements under "Filing of a Graduate Degree Curriculum". The dates above are deadlines for forms to Enrolled Services.

LATE FEE: A $25 LATE FEE WILL BE ASSESSED A STUDENT WHOSE GS2 OR GS4 FORM IS SUBMITTED THE DAY AFTER THE DEADLINE AND WILL INCREASE AT THE RATE OF $5 PER DAY THEREAFTER (EXCLUDING SATURDAY, SUNDAY OR UNIVERSITY HOLIDAYS). PAYMENT OF FEES MUST BE MADE DIRECTLY TO THE BURSAR'S OFFICE (BASEMENT OF SIKES HALL).
